This study aims to test the effectiveness of Integrating Islamic Psychotherapy Training (IIPT)—a combination of muhasabah–ṣabr-based assertiveness training and ta‘āwun-based family psychoeducation—on mental resilience, peer pressure resistance capacity, and family-caregiver involvement among students at Pondok Pesantren Miftahul Midad. A mixed-methods approach with a participatory action research design integrated into a quasi-experimental pre–posttest with control group design (N=64; 32 intervention, 32 waitlist control) was employed, complemented by data triangulation from in-depth interviews, FGDs, document reviews, and role-play observations. Results show significant improvements in resilience and assertiveness in the intervention group compared to control, with effects persisting up to a four-week follow-up. However, qualitative findings reveal a sharp knowing–doing gap: participants mastered formulaic concepts and skills, but execution collapsed under hierarchical pressure and cultural value conflicts, where spiritual reframing (jihādun nafs, qawl sadīd) proved to be an active mechanism reducing these barriers. Family psychoeducation increased knowledge but did not yet alter daily communication patterns. In conclusion, IIPT is differentially effective as a foundational intervention, but requires a follow-up supervision phase and reconfiguration of the family model to achieve behavior generalization. Implicationally, the integration of Islamic psychotherapy must be positioned as a restructuring of meaning rather than mere cultural adaptation, and pesantren policy should adopt it as part of spiritual formation.
